What to include in your request

  • Your full name and U.S. state of residence: required for eligibility verification.
  • The email address associated with your Healthcasts account (if applicable).
  • The right you wish to exercise: access, correct, delete, port, or opt out of sale/sharing/targeted advertising.
  • Sufficient information to verify your identity. We may follow up to request additional information.
  • If you are an authorized agent, please include written authorization to act on the requester's behalf.
